Approval of Minnesota Municipal Utilities Association contract for safety training and program
management services for 2018 at contract price of $12,600.00.

BACKGROUND/J USTI FI CATI O N :
The Safety Management Program contract with Minnesota Municipal Utilities Association (MMUA) expires
December 31, 2017. The attached contract continues services through 2018 including sixteen, 8 hour site
visits for a contract price of $12,600.00. Sixteen onsite visits is consistent with the current contract and
recommended by the safety committee.
Services provided by MMUA are focused on safety management and training including site visits to conduct
safety training, site reviews & audits, records management and safety program development.
The Safety Committee recommends contract approval with MMUA in 2018 including sixteen, 8 hour visits
with the contract price of $12,600.00. The Public Works Subcommittee reviewed the 2018 contract at the
September 13th meeting recommending approval.

AMENDMENT TO SERVICES AGREEMENT
Safety Management Program
Contract Date: August 30, 2017 Contract Number: 187-2018
The services agreement entered into between Minnesota Municipal Utilities Association
(MMUA) and the City of Otsego (Otsego), dated October 13, 2016, contract number
187-2017, is amended as follows:
PART I. Section 1.
2.d. SCOPE OF SERVICES: Visit Otsego for 16 visits (approximately 8 hours in a
one -day visit) from January 1, 2018 until December 31, 2018. The visits will be
made on Otsego premises unless circumstances lend a different location approved
by Otsego management.
PART II, Section 1.
1. DURATION: This Agreement shall remain in force from January 1, 2018 until
December 31, 2018 (the "expiration date"), as described in PART I, Section 1.
above.
PART III, Section 1.
1. COMPENSATION: For the services covered by this Agreement, Otsego shall
pay MMUA a fee of twelve thousand six hundred dollars and 00 cents
($12,600.00) for the annual period January 1, 2018 to December 31, 2018. Such
compensation shall be due and payable according to the selected payment terms
below.
Payment terms for the fees agreed to above shall be based on one of the
following options (select one):
❑ Annual Upfront Payment ($ 12,600.00)
❑ Quarterly Payments ($ 3,150.00 each)
For any term of less than twelve full calendar months, the fee shall be a portion
of the annual fee, pro -rated based on the number of calendar months or partial
calendar months in which the services were provided as a percentage of twelve
(12).
The parties hereby accept the terms of the Agreement as modified.
